Causes of lactose intolerance

Milk intolerance can have two causes: on the one hand, there may be a genuine milk allergy to the protein components of milk or it may be an enzyme defect in which the lack of the enzyme lactase leads to lactose intolerance.

What is the difference between a milk protein allergy and lactose intolerance?

  • Lactose intolerance should not be confused with a milk protein allergy, which is a genuine immune reaction. A milk allergy is normally directed against a protein component of milk. Depending on which component triggers the allergy, milk from another animal species (goat's or sheep's milk) or heated milk can be tolerated. If there is an allergy to the protein component casein, any type of milk should be avoided.
  • However, there are also allergies to components of heated milk - the so-called melanoidins, which are produced by heating sugar and amino acids). Raw milk is tolerated in this case. While patients with lactose intolerance can still tolerate small amounts of milk, allergy sufferers react to the smallest amounts of milk with symptoms. Interestingly, there is a very high number of lactose intolerances outside Northern Europe.

What to look out for?

  • Non-pasteurised yoghurt + fermented milk products still contain lactase activity
  • Fermented milk products contain very little lactose - can therefore still be tolerated by people with a mild lactose intolerance.
  • Whey is the dairy product with the most lactose, which is why practically all whey products are intolerable!
  • In hard cheeses, the lactose is almost completely broken down by bacteria during the ripening process. Hard cheeses are therefore considered practically lactose-free. The other types of cheese contain only small amounts of lactose and are usually very well tolerated.
  • Caution is advised with whey cheese and some processed cheeses with added milk powder.
  • As a general rule, the longer the cheese has had time to mature, i.e. the harder it is, the less lactose it contains.
  • Watch out for hidden sources of lactose in semi-finished and finished products: Sweets such as ice cream, chocolate, cream and caramel sweets, pudding, rice pudding, semolina porridge, margarine, bread and baked goods, crispbread, crackers & snacks, muesli, mashed potato powder, instant soups, soup seasoning, ready-made sauces and creams, salad dressings, frozen meat and vegetable preparations, tinned dishes, cooked and liver sausages, tinned sausages, medicines, sweetener tablets, food supplements
  • Make sure you have an adequate supply of calcium, vitamin B2 and vitamin D

Foods rich in calcium:

Cereal products, pulses, rice, black tea, figs, poppy seeds, sesame seeds

  • WINTER: beetroot, black salsify, kale, broccoli, banana, parsnip, kale, broccoli, courgette, apple (Jonathan, Crown Prince Rudolph, Golden Delicious)
  • SPRING: spinach, rowan, cranberry, blueberry, blackberry, blackcurrant (red and black), apple (Gloster, Boskoop), asparagus
  • SUMMER: Peas, watercress Foods rich in vitamin B2: Fish, poultry, mutton, veal, beef, pork, game, bread, almonds, hazelnuts, linseed, vegetables and mushrooms, apple (clear apples, Ida Red, Jonagold, Granny Smith), peas, spinach, mushrooms, chanterelles, porcini mushrooms
  • AUTUMN: Apple (Gravensteiner, Lobo, Mclntosh, Cox Orange) Foods rich in vitamin D: Sea fish, freshwater fish, veal, mushrooms, eggs

  • Caution: Probiotics are not better tolerated than normal milk!
  • Many patients with lactose intolerance also have fructose malabsorption. Fructose intolerance is often associated with depression. Lactose intolerance can further promote this development.
Alternatives to products (milk) that do not contain lactose:

  • Almond milk
  • Soya milk and soya desserts
  • Oat milk
  • Rice milk

Stir one level measuring spoon into food or drinks containing lactose, do not add to hot food (over 50°C).
