GO-ON® pre-filled syringes contain a sterile, pyrogen-free sodium hyaluronate solution. GO-ON is indicated for use as a viscoelastic supplement to the synovial fluid in the knee.

Sodium hyaluronate is a natural, high-molecular-weight polysaccharide composed of a linear chain of disaccharide building blocks consisting of sodium glucuronate and N-acetylglucosamine. Sodium hyaluronate is present in many tissues of the human body and constitutes the main component of synovial fluid, in which it is found in high concentrations. As a synovial fluid, it plays an important role in regulating the physical and mechanical interactions between adjacent tissues. As a viscoelastic support material, it also maintains the space between tissues. Sodium hyaluronate preparations may have different molecular weights but share the same chemical structure. GO-ON® is a 1% sodium hyaluronate solution obtained from Streptococcus equi via fermentation processes and subsequent purification.

Indications

GO-ON is indicated as a viscoelastic supplement to the synovial fluid in the knee. Viscosupplementation is recommended for patients of normal weight or who are moderately overweight with symptomatic mild to moderate knee osteoarthritis, whose condition cannot be adequately improved by non-pharmacological treatment and analgesics/NSAIDs, or who are unable to take analgesics/NSAIDs. The product has a lubricating and mechanically supportive effect and is suitable for the treatment of the symptoms of osteoarthritis.

Properties and mechanism of action

Synovial fluid, which is viscoelastic due to its hyaluronic acid content, is present in all synovial joints, particularly in the large joints, which are subjected to considerable stress due to body weight. Here, synovial fluid acts as a lubricant and shock absorber, thereby ensuring normal, pain-free joint movement. Furthermore, synovial fluid supplies the articular cartilage with nutrients. Degenerative joint diseases such as osteoarthritis are associated with a marked reduction in the viscosity of the synovial fluid, which impairs its lubricating and shock-absorbing functions. As a result, the mechanical stress on the joints and the loss of articular cartilage tissue increase to such an extent that pain and functional limitations occur in the affected joints. It has been demonstrated that an improvement in the quality of the synovial fluid, brought about by the intra-articular injection of sodium hyaluronate preparations, also enhances its viscoelastic properties. This enhances its function as a lubricant and shock absorber, which in turn reduces the mechanical stress on the joints. This results in pain relief and improved joint mobility. This effect can last for more than six months following a treatment course of five intra-articular injections.

Usage

  • GO-ON should be injected into the affected joint a total of 5 times at weekly intervals. Depending on the severity of the joint condition, the effects of a course of five treatments may last for 6 months. 
  • Treatment courses may be repeated if necessary. In the event of joint effusion, it is recommended to aspirate the fluid, immobilise the joint, apply an ice pack and/or administer corticosteroids via intra-articular injection.
  • Treatment with GO-ON® can be repeated 2–3 days later. Remove the syringe from the blister pack, remove the rubber stopper, fit a suitable sterilised needle (e.g. 19 or 21 G) and secure it by turning it gently. When attaching the needle and removing the needle guard, take care not to tighten it too tightly or apply excessive force. Before the injection, expel any excess air from the syringe. GO-ON should be administered separately and must not be mixed with other substances prior to injection.
  • Notes: This product must only be used by doctors. In particular, the doctor should be trained as an orthopaedic surgeon, rheumatologist or physiatrist.
